| Sunscreen Form | Must be printed and signed by the adult participant or signed by the participating child's legal guardian. Form must be turned in prior to the start of the first class. |
| Medication Administration Form (Optional) |
This form is required if the camper must use or has to bring medication during the camp day, such as an Epi-pen. It must be printed and signed by the medication prescriber, then by the participating child's legal guardian. Form and medication must be turned in to the Camp Director or their designee prior to the start of the first class. |
